InnovAge’s Program of All-inclusive Care for the Elderly (PACE) is an alternative to nursing facilities. Our clinics have worked through PACE for more than 30 years. InnovAge PACE is a program that provides seniors customized healthcare and social support at a nearby PACE center supported by a team of medical experts dedicated to providing personalized healthcare and support to help them age at home. Each PACE participant receives customized care that is planned and delivered by a coordinated, interdisciplinary team of professionals.

Guaranteed annual salary with no production!

Our Nurse Practitioner is a Primary Care Provider, you are responsible for the medical care of InnovAge’s enrolled geriatric participants by treating them primarily at the InnovAge Center clinic. You will perform Concierge-like clinical care, with the support of InnovAge’s Interdisciplinary Team (IDT).

Highlights:

  • Responsible for a patient panel of 95-100 enrollees

  • Minimal Call w/ RN Triage
  • Participate within IDT to formulate care plans.
  • Epic EMR

The Nurse Practitioner provide medical care to participants, including the evaluation, diagnosis, and management of their medical conditions and management of medical information from specialists and hospital teams. The Nurse Practitioner works under the direction of the Center Medical Director and/or the Statewide Medical Director and collaborates with the interdisciplinary team.

Required Qualifications:

  • Virginia NP License
  • 1+ Years experience practicing as a Nurse Practitioner
  • DEA Certification with full prescriptive authority
  • 5+ years a Registered Nurse with experience in elderly care

Preferred Qualifications:

  • Bilingual
