About the Facility: The Syracuse VA Medical Center, part of VA Health Care Upstate New York opened its doors on June 14, 1953. The Medical Center utilizes state-of-the-art technology to provide a full range of patient care services, education and research. The 106-bed general medical and surgical facility provides comprehensive: Primary care Mental health care Tertiary care Long-term care Physical medicine Rehabilitation Neurology Oncology Dentistry Geriatrics The Syracuse VA Medical Center also operates a Community Care Center that provides a community day program, mental health intensive case management program, homeless veteran program, and vocational services.The Department of Veterans Affairs named the Syracuse VA Medical Center as a Center of Excellence for its Operation Enduring Freedom/Operation Iraqi Freedom and Polytrauma programs. The Syracuse facility is also the principal referral center for neurosurgery and urological renal stone treatment, serving all of upstate New York and northern Pennsylvania veterans. Syracuse VA provides primary and behavioral health care in a 13-county area of Central New York through eight community based outpatient clinics in Auburn, Binghamton, Cortland, Massena, Oswego, Rome, and Watertown. The Syracuse VA Medical Center has an active affiliation with the State University of New York Upstate Medical University where over 650 of their students, residents, interns, and fellows receive training at the Medical Center each year. The Syracuse VA has eight affiliated nursing education programs and a physician assistant externship with LeMoyne College and a long-standing educational partnership with Syracuse University for field experience for audiology, speech pathology and social work students. Scope of Work: Direct Patient Care: 95% of the time involved in direct patient care. • Coordinating Interventional Radiologist(s) for all Interventional Radiology requests. • Determining the appropriateness of the requested emergent procedure(s) and activating the on-call Interventional Radiologist(s)as needed • Providing personal protection radiation monitoring equipment for contract Interventional Radiologists that conforms to State, FDA and VA National Health Physics Program standards for radiation safety • Ensuring that all contract providers maintain active computer access/verify codes and VPN access to maximize documentation requirements, place orders (verbal orders are not accepted), sign/co-sign documents virtually, and review patient information. • Entering preliminary results, physician post procedure notes, and orders immediately after every case. • Documenting all pre-procedural evaluations, consents (as necessary), procedural documentation, post procedure follow-up, and consultations in accordance with JC and VHA requirements. • Interpreting and signing all procedure notes within 24 hours of patient check-in. • Patient Safety Compliance and Reporting: Contract physician(s) shall follow all established patient safety and infection control standards of care. Contract physician(s) shall make every effort to prevent medication errors, falls, and patient injury caused by acts of commission or omission in the delivery of care. All events related to patient injury, medication errors, and other breeches of patient safety shall be reported to the COR VA Safety Policy. As soon as practicable (but within 24 hours) Contractors shall notify COR of incident and submit to the COR the Patient Safety Report, following up with COR as required or requested. • Scope of Care: Contract physician(s) shall be responsible for providing Interventional Radiology procedures, including, but not limited: o Angiography, Cryoblation, Balloon angioplasty/stent placement, Line insertion, Chemoembolization, IVC filters, Cholecystostomy , Vertebroplasty, Drain insertions , Nephrostomy placement, Embolization, Radiologically inserted gastrostomy, Thrombolysis , Dialysis access & related interventions, Biopsy, TIPS placement, Radiofrequency ablation, Biliary intervention, Endovenous laser ablation of varicose veins, Uterine Fibroid Tumor Ablations • Clinic: Contract physician(s) shall be present on time for any scheduled clinics as documented by physical presence in the clinic at the scheduled start time. • Consultation Responsibilities: Contract physician(s) shall provide consultation with and instruction to referring physicians regarding appropriate indications for interventional radiologic procedures so that the most expeditious and clinically appropriate work-up can be done. Contract physician(s) shall determine the appropriate course of treatment and communicate in person or by phone with the referring clinicians. o Contract physician(s) shall provide post-procedure evaluation and follow-up and clinical consultation regarding complications of interventional radiologic procedures. o Contract physician(s) shall ensure that consultative services are available in a timely manner in order to coordinate patient treatment with medical and surgical subspecialty services. o Provide consult referral and pre-procedure advice to residents and primary care clinicians • Medications: Contract physician(s) shall follow all established medication policies and procedures. No sample medications shall be provided to patients. • Discharge education: Provide discharge education and follow up instructions that are coordinated with the next care setting for all emergency department patients. ADMINISTRATIVE: 5% • QA/QI documentation: The contract physician(s) shall complete the appropriate QM/PI documentation pertaining to all procedures, complications and outcome of examinations. Qualifications: • Must have at least 1 year of experience. • Must have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia. License must be full and unrestricted. • Must be Board Certified. • Must have a current DEA Registration. • Must be able to speak, understand, read and write English fluently. • Must be a U.S. Citizen.
