Working either as the patient’s primary care provider or in an ‘extensivist’ role in coordination with a patients existing care team, USMM physicians deliver home-based medical care to the most complex and medically fragile patients who might otherwise go unrecognized and undertreated -- until they show up in an ICU.
USMM brings the full continuum of care to the patient: our core offering sends a physician and medical assistant team into the home to provide direct care to patients with complex chronic conditions or special needs associated with medical fragility or disability. We also offer clinical program services that conduct evaluations and assess patient needs, develop or modify plans of care, make referrals and manage special needs such as post-acute transitions of care or condition-specific clinical risks. All supported by a mobile EHR and clinical decision support software.
Our centralized 24/7 telephonic Patient Care Center provides access, communication and coordination services to patients, family members and all members of the patient’s care team. Our mobile laboratory, radiology and diagnostic services teams go to the patient rather than wait to schedule transportation and risk destabilizing their delicate balance of health -- and when skilled home health, hospice, podiatry, DME or pharmacy services are needed we bring those services to the patient. Weekly case conferences provide an opportunity to interact with peers and a physician governance program makes sure your voice is heard as operational decisions are made across the organization.
