1) Would pioneer a new specialty service/s offered by the facility. Provide insights for the creation of new practice discipline and assist in establishing protocol procedures.
2) In charge of providing timely and appropriate medical care for all patients.
3) Obtain detailed medical history from patient or care giver, perform physical exam, ordering and interpreting labs, potential diagnostic procedures and other testing as deemed necessary.
4) Consistently evaluate/improve the effectiveness of care provided to patient, in accordance with current clinic practice and professional medical judgment.
5) Completion of notes using EMR (practice fusion) accurately and in timely fashion.
6) Collaborate and provide advices/consultation to nurses, physical therapist, occupational therapist, caregivers, social workers, patient's relatives and office staff regarding patient's condition and plan of care.
7) Review patient/s records (EMR) and supervise various Physician Assistants and/or Nurse Practitioners that would be assigned to them.
8) Will be assigned to "go on rounds" to various locations
Locations: could be Assisted living facility, Nursing Homes, Hospices, Senior Living Facilities and Individual homes as need arises.
9) Responsible in signing pertinent documents for the management and patients well-being (T.O.'s P.O.'s medication refill, DME supplies, home health certifications and others).
10) Report directly to practice administrator for any concern/s.

*Current Indiana Licensure
*Current BLS & DEA
*Excellent communication, interpersonal and clinical skills.
*New graduates welcome to apply
