? Professional duties
? Full-time practice of Otolaryngology
? Inpatient, outpatient, and consultation
? Minimum of 32 Clinical hours per week
? Call 1:6, Thursday, 7:00 am to Thursday, 7:00 am
? Physician must perform all services at SwedishAmerican Hospital
? Must be Board Certified within five years of residency completion

? Facilities and support
? Office space, set-up, and practice management
? EPIC electronic outpatient medical record
? Clinical and clerical staff
? Marketing and practice promotion
? Established referral base of 60+ employed primary care physicians
? Dedicated audiologist team of 3 AuDs
? Academic affiliation with University of Illinois College of Medicine at Rockford
? Academic affiliation with University of Wisconsin at Madison

? Contract
? Employed by SwedishAmerican Hospital
? Three-year term with automatic renewal
? Termination of contract with 180-day notice by either party
? One-year restrictive covenant within 25-miles of practice site
? Must establish permanent residence within Winnebago or Boone counties

? Compensation & Benefits
? Two-year salary guarantee of $350,000
? WRVU-based productivity in subsequent years
? Malpractice provided during employment

? Three weeks vacation and one week CME during guarantee, unlimited after guarantee
