Maxim Government Services is seeking an Ophthalmologist for contract services at Fort Riley, KS, near Topeka, KS. Maxim will provide relocation assistance or housing. Contractors can work on any state license.

1.2.1. Qualifications.

1.2.1.1. Must possess a valid Medical Degree from an accredited School of Medicine or Osteopathic Medicine as well as successful completion at a formal and valid Ophthalmology residency training program. Must be either Board Certified by the American Board of Ophthalmology or the Osteopathic Board of Ophthalmology or be eligible to take the certifying exams. It is expected the physician is scheduled to take the exam if board eligible.

1.2.1.2. LICENSE/REGISTRATION. The contractor OPHTHALMOLOGIST shall possess a valid, unrestricted current license from any US jurisdiction and a current Drug Enforcement Agency (DEA) registration number (certificate) to prescribe controlled substances in accordance with the law of the state where services are provided. The licenses and certificate must be renewed and maintained in a current status during the life of the contract.

1.2.1.3. Must have at least one year of experience in refractive surgery.

1.2.1.4. Must be certified in the operation and use of the Visx Star 4 Laser in accordance with Federal Drug Administration requirements.

1.2.1.5. Must have some exposure to or experience in clinical research.

1.2.1.6. Have and maintain current Basic Life Support (BLS) certification. The American Heart Association Basic Life Support for Healthcare Providers is the only program accepted. BLS recertification shall be maintained in a current status at all times while the OPHTHALMOLOGIST is performing services under this contract.

1.2.1.7. Each OPHTHALMOLOGIST shall be able to obtain privileges and be credentialed (as appropriate) by the MTF’s Credentials Committee.

1.2.1.8. Each OPHTHALMOLOGIST shall have sufficient skills, knowledge and experience to perform the specific tasks (see Paragraph 5. of this Performance Work Statement) listed for their specialty.

1.2.2. Requirements.

1.2.2.1. The Contractor shall not employ any individual to provide direct health care services who is listed on the Health and Human Services (HHS) list of providers excluded from Federally funded health care programs. The list may be found at [Register to View] />
1.2.2.2. The Contractor OPHTHALMOLOGIST shall be a U.S. citizen or otherwise meet the requirements of AR 25-2, Information Assurance.

1.2.2.3. The Contractor personnel shall not have been convicted of a felony.

1.2.2.4. The Contractor personnel shall successfully complete a criminal history background check in accordance with paragraph 7.4. of the Performance Work Statement (PWS).

1.2.2.5. The Contractor personnel shall be able to read, write, and speak English well enough and enunciate with sufficient clarity to effectively communicate with patients and other health care providers.

1.2.2.6. Possess sufficient initiative, interpersonal relationship skills and social sensitivity such that the OPHTHALMOLOGIST can relate to a variety of patients from diverse backgrounds.

1.2.2.7. Able to operate a personal computer utilizing Microsoft applications, such as Word, Excel, and efficiently utilize an Electronic Medical Record.

1.2.2.8. The Contractor personnel shall use the various clinical automation systems as required, such as, but not limited to Composite Health Care System (CHCS), Armed Forces Health Longitudinal Technology Application (AHLTA), Ambulatory Data System (ADS), and Defense Medical Human Resource System – internet (DMHRSi).

1.2.2.9. Contractor personnel shall be required to meet Continuing Medical Education (CME) requirements in AR 351-3 or as determined by the provider’s State of licensure, whichever is more stringent. The Government will not reimburse for these costs.

1.4. Hours of Performance.

1.4.1. Regular. The contract physician will provide services for work weeks averaging 40 work-hours consisting of an average of five, 8-hour work days plus a 60-minute unpaid break, Monday through Friday, generally between the hours of 0730 and 1630. The Government will only pay for actual hours worked.

1.4.2. On-Call (Carry a Pager): This position requires the contract OPHTHALMOLOGIST to be on-call, both weekday, weekends and federal and training holidays. The on-call duties are shared by IACH ophthalmology physicians, optometrist providers and the contractor ophthalmologist. Call is generally performed four (4) times per month for a 24-hour period (i.e., 0700 to 0700 the following day - total of 1152 pager hours/year). While on call, the contract OPHTHALMOLOGIST will be required to carry a pager (beeper) and respond in accordance with current On-Call policies. The number of military ophthalmologists is subject to change due to mobilization, ETS, PCS, TDY, deployments, etc.

1.4.2.1. When the contract physician is on-call for weekends or holidays, the contract physician shall be required to perform rounds on the inpatient ward for any ophthalmology patients.

1.4.2.1. The OPHTHALMOLOGIST shall respond by telephone within 15 minutes after receipt of notification from MTF personnel. If attendance at the MTF is required, the OPHTHALMOLOGIST shall report for duty within 30 minutes of initial notification/page. The contract OPHTHALMOLOGIST will not receive additional compensation for being called back to the MTF to perform services.


1.4.3. The OPHTHALMOLOGIST may be required to work beyond the end of the shift while providing patient care in emergent situations. Extended time will be used for direct patient care only. The contract OPHTHALMOLOGIST will not receive additional compensation for working beyond his/her shift in order to accomplish administrative tasks. Signing in early does not constitute an extended shift.

1.4.4. The Contract OPHTHALMOLOGIST shall work a minimum of 1920 hours. Hours which may be worked to satisfy the 1920 minimum are Ophthalmology Clinic Support, overtime, and when called back to the MTF to provide care while on-call.

1.4.5. Absences. The contract physician is authorized a maximum of 15 working days, or a total of 120 hours of absences per year for sickness, scheduled vacation, emergency leave or other emergency and extenuating circumstances. All requests for absences due to vacation and CME/training purposes will be submitted in writing a minimum of 30 calendar days in advance to the Clinic/Department Chief or designee. The OPHTHALMOLOGIST shall notify the Department Chief or designee within one hour of scheduled reporting duty time if he/she is sick and cannot report to work as scheduled due to an emergency absence. No contract provider shall have worked at another institution during the shift preceding his/her assigned shift at IACH.
