Maxim Government Services is searching for a qualified Optometrist for our client, the White Earth Service Unit(Indian Health Services) located in Ogema, Minnesota. Services are needed Monday through Friday 8am-4:30pm for approximately 6 months. Any state license works. Maxim provides malpractice coverage and can assist with relocation and temporary lodging, if applicable.

The White Earth Service Unit provides healthcare services including family practice, internal medicine, pediatrics, dental, optometry with Joslin Vision Network for diabetic screening exams, physical therapy/occupational therapy, lab/radiology with mammography and bone density, pharmacy, mental health services, nutrition/diabetic educator services, and contract health services. The White Earth Service Unit currently has (1) fulltime optometrist; currently in an acting capacity as the facility Clinical Director. The purpose of this contract is to procure a contract optometrist for a period up to 6 months until a new Clinical Director is hired.

Contractor shall perform a full range of services in his/her specialty including the most difficult cases.
Services may include but are not limited to the following:

• Shall participate and complete orientation;
• Shall assist chief optometrist in training and oversight of optometric extern activities;
• Interview and collection of pertinent health data from patient;
• Review past medical history;
• Evaluation/assessment of patient condition and documentation in a legible manner;
• Institution of patient treatment plans;
• Assurance of necessary patient follow-up;
• Conduct case conference with IHS staff, patient, and/or guardian as necessary;
• Request diagnostic tests/examinations as deemed necessary;
• Arrange for specialized care and referrals for serious illnesses or trauma;
• Follow IHS guidelines for Contract Health Service (CHS) referrals;
• Shall be knowledgeable and perform services using infection control practices in accordance with accreditation standards, IHS policy and procedures, OSHA, and other regulating agencies and laws;
• Shall use personal protective equipment to protect patient and personnel from exposure to infectious disease as appropriate, e.g., blood borne pathogens, TB, MRSA, etc.
• Management of chronic health problems;
• Emergency eye care, diagnosis and treatment;
• Order medications from pharmacy as necessary;
• Perform diagnostic examination, screening and treatment of refractive error and ocular conditions/disease;
• Fully participate in optometry staff functions including peer reviews;
• Apply for and Maintain optometry staff privileges;
• Ability to work independently;
• Provides services in a professional and respectful manner when dealing with patients and colleagues;
• Shall respect and maintain the basic rights of patients including personal dignity;
• Shall meet all Medicare “Conditions of Participation” applicable to performance of this contract;
• Shall sign all necessary billing applications within 7 business days of entry on duty and maintain enrollment with Medicare, Medicaid and private insurers to allow for IHS billing;
• Other related duties as assigned.

Medical Records:

The Contractor shall record, in the patient's IHS clinical chart (medical record), through the use of appropriate IHS forms/EHR, the clinical findings of each patient visit, the provisional or confirmed diagnosis, services and treatment provided, disposition of the patient and instructions given to the patient and/or family member (identify family member by name and relationship to patient). The documentation must be legible and must meet the requirements of the service unit, medical records and medical staff policies and procedures, and of accreditation standards.

In addition to providing optometry services the contract provider shall be able to meet the following criteria:

1. Degree: Doctor of Optometry (O.D.) accredited by the Council on Optometric Education.
2. Experience: Shall have been in active practice for the past one year (12 months).
3. Licensure: current and unrestricted license to practice in a State, District of Columbia, or territory of the United States.
4. DEA Certificate: possess and maintain current certificate when applicable.
5. Certification: BLS
6. Continuing Education: shall provide proof of continuing education for the past 12 months.
