Maxim Government Services is searching for qualified BE/BC Ophthalmologists for our client, the Minneapolis, MN Veteran’s Affairs Medical Center. Services are needed Monday through Friday 8am-4:30pm. The Minneapolis VA Healthcare System (MVAHCS) requires contractual locum tenens services of up to four full-time Ophthalmologists to provide ophthalmology clinic and surgery services for eligible veteran beneficiaries of the Minneapolis VAHCS. Ophthalmology services required include outpatient clinic, inpatient attending, consultative, operating room surgical services, retinal reads, training and supervision of residents and medical students, and on-call coverage. Services shall be provided for the Surgical & Specialty Care Patient Service Line.

Ophthalmologists are required who can provide general ophthalmology services and/or ophthalmology subspecialty services. It is anticipated that there will be multiple providers. Subspecialty providers are required who have training in an ophthalmology surgical subspecialty, i.e.: glaucoma; strabismus; ocular plastics; neuro-ophthalmology; retina; or corneal/anterior segment.

Ophthalmologists provided should be willing to participate in the training of residents and medical students. The MVAHCS is an affiliated teaching center. Supervision and education of residents and medical students are significant aspects and key requirements.

Services are required for one 3-month base period with three 3-month option periods.

Services will be provided within the Minneapolis VA Health Care System at the Minneapolis VA Medical Center, One Veterans Drive, Minneapolis, Minnesota 55417.

Qualifications:

Personnel assigned by the contractor to perform the services covered by this contract shall have a full and unrestricted license in a State, Territory or Commonwealth of the United States or the District of Columbia, and shall meet the professional qualifications criteria of VA. The qualifications of such personnel shall also be subject to review by the MVAHCS Chief of Staff and approval by MVAHCS Director.

Ophthalmologists provided must be board certified/board eligible by the American Board of Ophthalmology. All ophthalmologists who will be performing subspecialty surgeries must have formal fellowship training in a specific subspecialty, i.e.: glaucoma, retina, corneal/anterior segment, etc.

Physicians provided must have the technical proficiency and expertise to provide a broad range of clinical care and related ophthalmology clinical and surgical treatments to patients receiving care from the VA Ophthalmology Service inpatient and outpatient programs. Contractor must be capable of coordinating and providing professional guidance to ensure that the appropriate treatment methodology is set forth for each patient admitted to the Ophthalmology Service for eye problems, including pre-operative evaluations and post-operative outcomes.

Contract employees shall complete all continuing medical education on a timely basis as required to maintain American Board of Ophthalmology board certification/board eligibility.

ACLS/BLS: It is a policy of this medical center that all contract and sub-contracted personnel to be employed under this contract must be Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS) certified unless a waiver has been granted by a Patient Service Line (PSL) Director. As part of the credentialing process, the contractor’s ACLS/BLS certifications must be sent to the Medical Staff Office. Failure to provide proof of certification to the Medical Staff Office may impact the contract personnel qualifications under this requirement. No services will be provided by any contract personnel prior to obtaining approval by the VA Medical Center Professional Standards Board, Clinical Executive Board and Director.

CPRS: Contractor employees (including any sub-contract employees) providing medical services will be required to attend Computerized Patient Record System (CPRS) training prior to providing any patient care services, including on-call/emergency coverage at VA. The contractor employee will document patient care in CPRS to comply with all VHA and Joint Commission standards.


The National Provider Identifier (NPI) is a standard, unique 10-digit numeric identifier required by HIPAA. The Veterans Health Administration must use NPIs in all HIPAA-standard electronic transactions for individual (health care practitioners) and organizational entities, i.e. medical centers, clinics, etc. The contractor shall ensure that the healthcare practitioners and/or medical center providing service under the contract obtains a NPI and provides it to the contracting Officer.

Contract physicians will be required to provide their own Drug Enforcement Administration (DEA) certificate number for prescribing controlled drugs/medications.

Hours of Operation:

The VA Medical Center is open 24 hours per day 7 days a week 365 days per year. Administrative offices, clinics and the OR general hours of operation are Monday through Friday 7:00AM – 4:00PM. The services covered by this contract shall be furnished by the contractor as defined in the work schedule below. The contractor shall also be required to furnish emergency on-call services on a national holiday and during off-duty hours. (See below paragraph 4.1- Services Required)

Work Schedule

Clinic Schedule: Ophthalmology Clinics are scheduled Monday through Friday every week, excluding federal holidays, and run from 7:30am – 4:00pm or until last patient is seen. Extended clinic hours may be required due to patient access.

Operating Room Schedule: Based on the current Ophthalmology Service surgery workload, operating room (OR) surgical requirements are:
• There is one operating room available Monday, Tuesday, Thursday, Friday, and there are two OR suites available on Wednesdays. Ophthalmology surgical cases are scheduled between the hours of 7:00am and 4:00pm.
• OR schedules will be determined on a weekly basis (with the exception for emergency cases) and will be communicated through the Ophthalmology Administrative Office.
• On surgical days with first case starts at 7:30am, the surgeon must sign in by 7:00am on the OR log in the OR control room, with documentation completed by 7:15am.


Retina Imaging Schedule: (This service is preferred but is not required.) Contractor will provide ophthalmologist(s) dedicated to read, interpret, and report on retina images for the Minneapolis VAHCS and for images sent from eleven other VA sites. It is estimated that between 600 – 800 images will be read per month. It is estimated that this will require ophthalmologist working approximately 20 hours per week.

On-Call Schedule: Contract physicians will be required to participate in after hours
on-call services. Call coverage will not exceed more than one out of every four weekdays and one out of every four weekends. Monthly call schedule will be provided by the Ophthalmology Department.
Call coverage hours are as follows:
• Weekday call coverage (M-F) starts at 4:00pm and goes until 7:00am the following morning
• Weekend call coverage is from 7:00am on Saturday to 7:00am on Monday
• Holiday call coverage is from 7:00am to 7:00am the next day
• Weekend call is estimated at 156 hours per 3 month period
• Weekday call is estimated 244 hours per 3 month period


Contractor must be present on-site at VMAHCS and actually performing the required services for the periods specified in the contract. Contractor is required to submit a monthly timesheet to validate presence and on-call services. The contractor will invoice his time only for the actual hours providing services to the MVAHCS.

Contract physician may not work at another facility during normal VA tour of duty as defined above.

Services Required:

The Minneapolis VA Health Care System (MVAHCS) requires 24/7 Ophthalmology Service departmental coverage for the services of experienced, practicing Ophthalmologists to provide general clinical services as well as Ophthalmologists with subspecialty surgical expertise.

The contractor services may include, but are not limited to:

Staff outpatient clinics, inpatient attending, consultative services, surgical procedures, reading of retinal images, training and supervision of residents and medical students, and on-call coverage. These services are not limited to one provider. It is anticipated that these duties will require the services of several Ophthalmologists.

In cases of medical necessity, when patients are unable to receive required treatment at the MVAHCS due to the lack of equipment or staff not available, patients may be referred off-site for care. Any referral for services outside of the VA will be processed through the MVAHCS Ophthalmology Clinic Coordinator and are not considered a part of this contract.
Staff are to be present in the clinic from the start to the finish of each clinic. Residents or fellows will not start a clinic without staff present.
Contractor may be required to read and report on retina images. All work provided for the retina imaging program shall meet timelines for reading and reporting of retina images. The MVAHCS turn-around-time for retina reads is one week. Total Turn-around-time (TAT) is defined as the interval between the times the images are obtained and the time the final report is visible in the VA’s Computerized Patient Record System (CPRS).
Contractor physicians provided shall have appropriate management skills, consultation skills, and clinical skills, that reflect community standards with capability to perform a broad range of ophthalmology clinical care and surgical procedures of varying complexity as required to maximize effective patient care for veterans admitted to the MVAHCS Ophthalmology Service inpatient and outpatient programs.
Contract Ophthalmologists shall coordinate, provide professional guidance, and ensure that the appropriate treatment methodology is set forth for each patient admitted to the Ophthalmology Service for surgery, including pre-operative evaluations and post-operative outcomes. Contract surgeon will see all patients prior to an operative procedure.
Technically directs surgical cases in a close team care framework. Secures and assures similar technical direction by qualified ophthalmology specialists for surgery workload and in cases requiring ophthalmology subspecialty expertise.
Provide consultative services, evaluation and initial treatment in an organized and properly directed manner, consistent with the defined capability of the VA Medical Center. Contractor Ophthalmologists must review consults and provide a response in CPRS with either a treatment plan or consult determination within seven days of receipt of consult.
Contract provider’s standard of care shall cover the same range of services as would be provided in a civilian medical treatment facility, and shall follow the standard of care required by the MVAHCS. Service performance will be satisfactory as defined by the VA National Surgery Quality Improvement Program (VASQIP) range for mortality and morbidity in this specialty. The Ophthalmology Service Chief will review surgical cases to ensure practice reflects required standards and will provide a monthly quality report addressing morbidity, mortality and complications to the Surgery & Specialty Care PSL Director.
Complies with all appropriate and applicable policies, procedures and regulations of the VHA and MVAHCS to ensure compliance with Joint Commission guidelines, VHA Resident Supervision Handbook 1400.01, and adheres to documentation levels that are standard in the medical community, i.e. Center for Medicare and Medicaid Services (CMS).
Ophthalmologists provided will possess collaborative skills sufficient to work effectively with physicians from referral departments/specialties and to cultivate collaborative relationships.
Contract physicians must maintain professional conduct and the highest standard of integrity during interactions with patients, patients’ families, nurses, physicians and other healthcare professionals. Contract physicians will consistently communicate and treat others in a courteous, tactful and respectful manner.
All patient care shall be appropriately documented in medical records in accordance with standard commercial practice and guidelines established by the MVAHCS. Documentation in CPRS will be within Medicare compliance and to be compliant with VA Resident Supervision Guidelines, >90% compliance for all resident supervision documentation requirements for inpatient and outpatient notes. The Minneapolis VAHCS will not provide payment for contractor services until contractor has satisfied all medical record documentation requirements for CMS and GME guidelines. (See 4.4.4. Professional Standards for Documenting Care.)

Outpatient Clinic Coverage:
Ophthalmology Clinics are held Monday through Friday from 7:30am to 4:00pm. The rolling average is 120 – 140 patients seen per day. Clinic schedules are subject to change based on clinic access demands.
New patients must be seen within 14 days of desired date. If access requirement are not met due to circumstances beyond contractor’s scope, the requirement will be reviewed with the Surgery/Specialty Care PSL Director.
When contract Ophthalmologists are scheduled to be in clinic, they must be on time and physically present for all scheduled clinics and ensure that patients are seen within a reasonable length of time.
For resident clinics, staff must be present in the clinic from the start to the finish of each clinic. Residents or fellows may not start a clinic without staff present.
All required documentation, i.e. CPRS clinic note and encounter are completed on clinic day. (See paragraph 4.4.4 – Professional Standards for Documenting Care)

Operating Room Coverage:
The operating room schedule for Ophthalmology Surgery is Monday through Friday with one operating room available daily except Wednesdays when there are two O.R. suites available. On surgery days with first case starts, surgeon must be onsite in the O.R. by 7:00am with an operating start time of 7:30am and pre-operative documentation completed by 7:15am.
The Ophthalmologist who performs the surgery must see the patient prior to the operative procedure.

Inpatient Coverage:
Contractor is responsible for the clinical care and related surgical treatment of veterans admitted to the Ophthalmology inpatient service. Contract Ophthalmologist monitors patients post-operatively and documents outcomes. A CPRS attending staff note is required for all inpatients within 24 hours of admission. Subsequent progress notes documentation is required during the hospitalization to document any significant changes in the patient’s condition and to demonstrate appropriate resident supervision.
