Home-based primary care practice serving Chicago, Cook County, and the surrounding areas. With an office culture that is welcoming and supportive, the leadership has a strong focus on work/life balance. The practice is well-established and as much flexibility as possible is given to all providers.

The practice's mission is to bring healthcare to those who are most in need and have difficulties travelling to a provider's office. Through the practice and its partner resources, patients are able to receive primary care visits, routine medical procedures, lab testing (blood draws), x-rays, ultrasounds, medical equipment, and much, much more, all in the safety and comfort of their own homes.

POSITION DESCRIPTION:
Serve as a primary care provider for a dedicated patient base of complex geriatric patients. You will see your patients as often as you deem necessary, which is typically one (1) visit per month. All are homebound or have limited mobility and benefit from receiving care in their own residences. You will provide the fullest scope of care possible, with the ability to evaluate your patients on medical, social, etc. levels, which is difficult to experience in an office-based setting.

Travelling to your designated territories, you will see an average of 12-14 patients per day, conveniently located to each other as scheduled by the office’s highly-trained team, who use mapping systems to ensure a route that makes sense. As often as possible, you will begin and end at your home. Only minimal visits to the office are required for signatures, staff meetings, etc.

You will utilize a highly-supported mobile EHR and communicate with your team via phone. This technology will allow you maximum flexibility, while providing the highest possible level of administrative and managerial support.

All orders can be placed through the EHR and will be carried out by your assigned coordinator. Your focus will be solely on seeing your patients and providing excellent care, while the office staff attends to all administrative and operational aspects.

There are minimal on-call duties; where on-call is necessary, it will be phone triage only and calls will be limited. The practice’s management team is happy to work with providers in scheduling on-call duties around vacations and other personal commitments.

REQUIREMENTS:
valid state (Illinois) medical license or ability to obtain one in a reasonable timeframe
current DEA or ability to obtain one in a reasonable timeframe
exposure to geriatric care preferred, but not required
