Hours: 8 hour shift, Day shift
About the facility:
Services include:
• ShortStay care
• Neurological care and rehabilitation
• LongTerm care
• Hospice care
• Cardiac Management Program
• Pulmonary Program
• Transitional Care Unit
• Attending Physicians
About the Provider Duties:
• Visits the patient at least twice weekly.
• Calls the responsible party/next of kin as well as referring provider shortly after admission to introduce themselves and to discuss the plan of care.
• Responds promptly (within 15 minutes for non-emergent calls) to the facility nurses.
• Promptly addresses any patient with an acute change in condition.
• Makes themselves available to the patient and/or family Physician or NP/PA, when possible, assesses the patient within 4 hours of admission to the center members at all times.
• Treats patients and their family members with compassion at all times.
• Provider will have Medical Directorship responsibility
