High Risk Medical Homes treat patients with multiple chronic illness. We are looking for experienced physicians to manage patients that are failing in their traditional primary care setting. Intercede Health-s High Risk Medical Homes provide a different level of care for patients. We see each patient for an extended weekly visit and really get into total care coordination, chronic disease management, and social factors that our affect our patient-s daily lives. Since we provide such detailed and personal care our physicians see an average of 8 patients per day instead of 4 patients per hour in your typical primary care medical home.

We have a new practice locations being developed in Beaumont and Port Arthur, TX.

No weekends
