Physician Bonus Structure
1. Incentive Compensation: As incentive compensation, Hospital agrees to pay physician those amounts that exceed the base salary according to the following formula:

a. Clinic Services: Because of the benefits of economics of scale, the Hospital will increase the Physician’s compensation as follows:

If collections during the quarter is less than $19,000, 43 percent of the amount collected will be applied to an incentive bonus. If the average monthly collections exceed $19,000 and is less than $22,000, 44 percent of that amount will be applied to the incentive bonus. If the collections exceed $22,000, 45 percent of that amount will be applied to the incentive bonus.

b. Emergency Room Services: For each day Physician provides emergency room services Monday through Friday, Hospital will add $90 to the incentive bonus.

For each Saturday or Sunday that Physician provides emergency room services, Hospital will add $200 to the incentive bonus.

In addition, 55 percent of Physician’s gross billings for emergency room services will be applied to the incentive bonus.

Incentive Bonus: Hospital shall pay to Physician as an Incentive Bonus, the amount by which the incentive bonus credit referred to in Paragraphs a, and b hereof exceed (All cash left over) the sum of $37,500.00 per contract quarter. Payments will be made to Physician within 30 days of the end of the contract quarter.

“Collections” shall mean all collections received for medical services provided by Physician during the term of this Agreement.

2. Coverage Schedule. Physician shall devise, maintain, and revise a schedule of rotation and coverage for the department in conjunction with the other providers providing services in the department.

It is the intent of the Hospital to have sufficient Providers to have a normal rotation of one in three, including weekdays and weekends. In the event this does not occur, the following payment schedule shall apply:

With the exception of planned absences of other physicians, including vacation, illness and CME, any coverage that is more than every third day and every third weekend shall be reimbursed at $60 per hour for weekdays (during non-Clinic hours) and $60 per hour for weekends. This amount shall be above and beyond the base salary guarantee.

3. Dues/Subscriptions:$1500 per contract year.
4. Continuing Medical Education:$2000 per contract year.
