PERFORMANCE WORK STATEMENT ADDENDUM
INTERNAL MEDICINE PHYSICIAN SERVICES
AT TRIPLER ARMY MEDICAL CENTER, HI

5.0. SPECIFIC TASKS – DESCRIPTION OF WORK
5.2.1. Scope of Services. Serve as a practicing internist responsible for the care of patients referred to the Internal Medicine Patient Centered Medical Home (PCMH). Scope of services includes, but is not limited to, examination, diagnosis, and treatment of patients referred for or currently being followed by the Internal Medicine PCMH Service for confirmed or suspected internal medicine problems. Participate as a member of an integrated clinic practice and serve as a primary care physician responsible for the primary health care of a panel of up to 1,000 patients or their equivalent (panel size will be adjusted to account for the number of patients age 65 years and older because these patients have a higher utilization rate for clinic services). The HCP shall possess professional knowledge of developmental pediatric principles and concepts sufficient to perform clinical and diagnostic services and shall be knowledgeable of and have the ability to develop individualized treatment plans.

5.2.2. Position: Internal Medicine Physician.
5.2.3. Assigned to: Internal Medicine PCMH, Department of Medicine, Tripler Army Medical Center
5.2.4. The HCP shall possess a Doctor of Medicine degree (M.D.) from an accredited college approved by the Council on Medical Education and Hospitals of the American Medical Association or American Osteopathic Association, or Doctor of Osteopathy degree (D.O.) from a college accredited by the American Osteopathic Association or possess permanent certification by the Education Council for Foreign Medical Graduates (ECFMG) or completed a Fifth Pathway Program.
5.2.5. Must be Board Certified by the American Board of Internal Medicine and have completed an applicable internship and residency in an accredited U.S. Medical Specialty Training Program.
5.2.6. Must have and maintain a valid, unrestricted license from any U.S. state, District of Columbia; or U.S. territory or jurisdiction to provide health care within the scope of practice for a specific health care discipline. In addition, contract physicians who write prescriptions for controlled substances must have a current Drug Enforcement Agency (DEA) certificate or Controlled Drug Substance number as appropriate. The contractor shall ensure that renewal actions are accomplished well before expiration of the provider’s license and/or DEA certification to ensure compliance with the contract. DEA certificates are not necessary at overseas locations.
5.2.7. CARDIOPULMONARY RESUSCITATION (CPR). Must possess and maintain current course completion documentation for basic cardiac life support (BLS) through an American Heart Association approved training program. Online or any other CPR certification will not be accepted. This certification shall be maintained in a current status at all times in order to perform services under this BPA call. While Advanced Cardiac Life Support (ACLS) or other advanced life support training may also be required for specific positions, they will not be accepted as a substitute for the BLS requirement.
5.2.8. REQUIRED DOCUMENTATION. The HCP shall provide the following documents pertaining to the contract with its offer for each healthcare provider submitted for consideration: Curriculum Vitae or resume demonstrating the qualifications and experiences set forth above in English, Letters of recommendations, performance appraisals, and any other documents that reflect on the contractor’s ability to fulfill the requirements of this BPA Call.
5.2.9. A favorable background investigation must be completed before services may begin. The completion of a Federal Bureau of Investigation (FBI) fingerprint check with favorable results and the successful submission of a National Agency Check with Inquiries (NACI) (equivalent or higher) background investigation to the Army’s investigative service provider, the U.S. Office of Personnel Management (OPM), via the U.S. Army Personnel Security Investigation Center of Excellence. The contractor shall be responsible for providing the required documents to the COR.
5.2.10. Must be able to pass and maintain a Criminal History Background Check. The contractor will be required to sign a statement consenting to a criminal history background check.
5.2.11. Experience: The HCP shall possess the knowledge skills and abilities to provide internal medical clinical services and shall have at least 6 months of experience in caring for patients as an Internist within the past 12 months.
5.2.12. The HCP shall have sufficient experience to be able to perform and interpret medical procedures to include, but not limited to:
- EKGs
- Treadmill Stress Tests
- Thoracentesis
- Paracentesis

- Lumbar puncture
- Pulmonary Function Tests
5.2.13. The HCP shall have sufficient experience to be able to perform tasks to include, but not limited to:
- Communicate with referring physicians and consultants appropriately
- Provide medical advice over the telephone.
- Write preliminary Admission Orders
- Complete all required paperwork
- Perform outpatient internal medicine consultations during scheduled duty hours, to include
comprehensive preoperative consultations
- Obtain an adequate history, physical, assessment and plan in a courteous, timely and appropriate manner
on each patient and develop an appropriate treatment plan.
- Utilize paraprofessional staff as appropriate
- Educate and counsel patients and families/significant others regarding preventive and treatment aspects of
health care
- Provide psychosocial and therapeutic support to patients and families/significant others to facilitate
adaptation to illness, death and dying, and lifestyle changes for health risk reduction.
- Supervise housestaff, physician assistants, and nurse practitioners
5.2.14. The HCP providing services under this contract shall perform the same clinical duties as those required of any military or government civil service physician of similar experience assigned to the same unit.
5.2.15. Perform active duty sick call and write temporary medical profiles for military personnel as required.
5.2.16. Consult with other healthcare providers, e.g., primary care physicians and specialists, regarding evaluation, assessment, diagnosis, treatment, and follow-up of patients who are cared for by the Internal Medicine PCMH Service. Provide professional advice to TAMC clinical personnel and/or other department/service/clinic staff with regard to professional management of patients being treated.
5.2.17. Make telephone calls to patients or to other health care professionals to provide patient care, consultation or medical management services.
5.2.18. Prepare all records and reports as may be required in support of services rendered and as required by the MTF. This includes special reports and forms such as medical boards and profiles, insurance forms and family housing exceptions. Dictate consults and procedure reports as required. Make appropriate entries in patient health care records and sign or electronically authenticate the entries. Ensure patient documentation and reports are legible. Verify the content and correctness of all prepared patient documentation and reports by validating its contents prior to signature or electronic authentication. Correct any errors or omissions in patient documentation and reports.
5.2.19. Facilitate patient access to medical care by initiating appropriate referrals to other health care providers. Follow MTF policies and procedures when referring a patient or requesting a consultation. The MTF must approve any referrals or consultation requests with providers outside the MTF.
5.2.20. Become familiar with the TAMC formulary and follow TAMC’s established procedures when prescribing drugs.
5.2.21. Use CHCS/AHLTA and other MTF automated information systems as required to (1) treat and manage patients, (2) document patient care, and (3) communicate with health care providers and ancillary staff. When necessary, utilize the appropriate hardcopy form if CHCS/AHLTA or other MTF automated information systems are not operational. Make appropriate computer entries in the CHCS/AHLTA Ambulatory Data Module (ADM) data entry system to document patient diagnoses, evaluation and management CPT codes, CPT codes for procedures performed, and the patient’s disposition. ADM entries shall be completed within 3 days of the patient encounter.

(The MTF may require the physician to complete a hard copy form (“Super bill”) in lieu of making entries in the ADM data entry system. The Patient Administration Division’s coding section will code encounters in ADM for Veteran’s Affairs beneficiaries and patients with other health insurance. The Patient Administration Division’s coding section will also code encounters classified as observation visits in ADM on behalf of the contract physician.)
5.2.22. Document time worked in the automated Work hour Tracking System in a timely manner. This system is used by the MTF to allocate personnel expenses in order to compute the cost of services.
5.2.2.23. Participate in physician peer review activities. Peer review will not be scheduled for more than 1–2 hours monthly. Peer review of charts performance shall meet or exceed clinic standards as set forth by the Chief, Adult Medicine Clinic or his/her designated representative.
5.2.24. Duty Schedule
5.2.24.1. The duty schedule will be determined by the Department Chief of their respective assignment with approval by the COR. The HCP shall provide services 40 work-hours per week, Monday through Friday, between the hours of 7:30 am – 4:00 pm, plus a 30 minute unpaid meal break. The government will pay only for actual hours worked. When required to ensure completion of services that extend beyond the normal close of business, the HCP shall remain on duty in excess of the scheduled shift.
5.2.24.2. A 30 minute unpaid lunch period is authorized and shall be scheduled by the HCP and coordinated with the Department Chief in order to provide continuity of operations throughout each day. The number and length of breaks shall be scheduled by the HCP and coordinated with the Department Chief. Hours worked do not include travel time to reach the military treatment facility (MTF) or other place of performance.
5.2.24.3. The Government reserves the right to review the hours worked by the HCP by any means necessary, including sign-in/sign-out procedures or by requiring the Contractor personnel to utilize a time clock should one be available as a method of quality assurance.
5.2.24.4. Overtime services are not required.
