Opportunity:
As the incoming Geriatric Medicine Physician, you'll join an established group in the SF and Bay Area working exclusively in skilled nursing facilities. You'll work days with multiple facilities providing “bread and butter” inpatient/geriatric medicine to elderly patients.
Duties may include:
- perform evaluations, including H&P’s, and orders at the time of admission
- oversee and participate in daily rounds
- perform duties of patient's primary-care physician
- manage communication among doctors plus medical team, patient and their family
- coordinate care with/among Specialists
- provide pre-op evaluations
- provide consultations
- act as transition coordinator
- collaborate with Case Managers
- provide discharge summaries and referrals
- schedule patient follow-up
- support every member of the team as required
- other geriatric medicine duties as necessary
Compensation & Location:
