Geriatrician needed for Boston based PACE program, an innovative, comprehensive, capitated program designed to maintain frail elders in the community. The program is one of only 42 in the country and offers a unique opportunity to work with a collaborative interdisciplinary team to manage the care of very frail elders. As a Physician in the program, which is both the insurer and the provider of care, you are empowered to design the best plan of care for your patients. The interdisciplinary team provides you with on-site collaboration and support for their care. Due to the frailty of the patients and the need to closely monitor their care, the caseload averages 150-200 patients and is shared with two nurse practitioners. We offer comptetive salary and an excellent benefits package. Massachusetts MD or DO license, Geriatric fellowship or experience and strong interest in working with the Geriatric population.
They are part of a private non-profit corporation that provides a wide variety of healthcare services. This program opened in 1996. The majority of services are provided at the Adult Day Center in Dorchester. This gives our participants the opportunity to socialize and get their health care needs met in one location. Services are also provided in the patients’ residence including private homes, hospitals, assisted living facilities, or nursing homes if needed. Our approach is personalized, respectful and flexible. We have grown quickly and continue to do so through word of mouth; our clients and their family attest to the excellence of care which we provide each and every participant.
