We are seeking physicians to provide care to medically fragile patients in their homes.
We work with a wide spectrum of patients all of whom need a higher level of care.
Our goal is to allow our patients maximum function while remaining in their own homes
Our continuous chronic care management, coordination, and oversight ensure appropriate monitoring of these patients between critical events.
The ideal candidates for this position will be board certified in internal medicine and fellowship trained in gerontology.
We provide extensive support to our physician making home visits, and offer an excellent salary and benefit package including health care, paid holidays and paid time off (PTO), 401K plan , and malpractice coverage with tail.
