The Huntington VAMC is seeking a Medical Director for our Home Based Primary Care (HBPC) program. The Physician will report to the Assistant Chief of Staff for Geriatrics and Extended Care. The incumbent is responsible for the overall medical care delivered by the HBPC team. Candidate serves as a consultant to HBPC staff for patients with complex medical problems and treatment needs in the home. Other area of responsibility will include, but not limited to: Provide clinical input and oversight for all patient treatment plans. Apprise the HBPC team of medical care advances and practice standards. Plan and direct educational experience of medical students, residents, and fellows assigned to HBPC. Make episodic home visits based on the needs of the patient and staff. Effective interaction with related services within the health care center. Maintain a high degree of excellence in clinical outcomes while providing patient care. Work in a team setting to perform duties to accomplish the treatment of patients.
