Summary of Job Duties
Duties will be on-site, to perform physical exams, prescribe medications, review x-rays and lab reports, etc. Records a summary of the problems and symptoms, diagnosis, treatments, medications and other appropriate info in patient’s medical chart. Makes referrals to specialists when necessary. Use on-site clinic space during scheduled visits.
Licensure/Certification
FP, IM or GP physician must have degree from an accredited medical school. Copy of graduate medical license must be provided. Must possess current, valid license to practice medicine in Virginia. Must be BC if FP or IM, and have valid DEA certification and valid NPI.
Dates
This contract will start upon credentialing for approximately 2 months, or possibly longer. Will work approximately 15 hours per week, schedule to be determined between provider and facility (flexible).
