About The Facility: The Canandaigua VA Medical Center, part of VA Health Care Upstate New York is located in the heart of the Finger Lakes region, 30 miles southeast of Rochester in Canandaigua, New York. Providing inpatient and outpatient care to veterans living in upstate New York since 1933, the Medical Center provides numerous health care programs and services. Scope of Work: • Required to provide primary care for a panel of patients in a Primary Care Aligned Team (PACT) at the Canandaigua VA Medical Center or the Rochester VA Outpatient Clinic (ROPC), 465 Westfall Road, Rochester, NY 14620. Both sites are approximately 30-miles from each other and the Government reserves the right to determine which location a provider will work at. • Each VA PACT team will be staffed by the temporary physician, one Registered Nurse (RN), one Licensed Practical Nurse (LPN), and one Medical Support Assistant (MSA). • The physician will evaluate and treat a total of approximately 10-12 patients per day. • PACT teamlets have additional support provided by Clinical Pharmacists, Medical Social Workers, Dieticians, and Integrated Mental Health Social Workers. • Timely evaluation and treatment of patients • Complete all documentation and close all patient encounters in a timely manner, by the end of shift, utilizing the Computerized Patient Record System (CPRS) • Order and monitor the results of diagnostic testing, notifying or directing the notification of patients regarding test results within 14 days of receipt • Order and adjust appropriate medication • Provide and document patient education as clinically needed • Monitor clinical response to treatment and adjust levels of care as necessary • Monitor and differentiate patient type of medication effects/side effects Qualifications: • Must have at least 1 year of experience. • Must be Board Certified or Eligible in Internal Medicine or Family Medicine. • Must have a current, full and unrestricted license to practice medicine in a State, Territory or Commonwealth of the United States, or in the District of Columbia. • Must have a degree of Doctor of Medicine or an equivalent degree resulting from a course of education in Medicine. The degree must have been obtained from one of the schools approved by the Secretary of Veterans Affairs. • Must have current BLS Certification (NOTE: ACLS is not a substitute for BLS on this contract). • Must be a U.S. Citizen. • Must be proficient in spoken and written English. • Must be proficient in the use of computers. • Must not have any physical limitations related to walking, bending, lifting, standing, or typing.
