Continued growth at Community Health Centers has created opportunities for three Family Practice Physicians to join our group. We are the predominate primary care provider and currently provide approximately 80% of the primary care in the service area. We continue to experience considerable growth and currently employ approximately 45 Providers at 6 modern medical practice sites in central Vermont and provide approximately 140,000 visits to almost 40,000 patients annually.

All sites are certified Level lll Medical Homes, with a highly developed and supportive panel management and case management system in place. We also have a robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within our primary care sites.

We have a fully integrated electronic medical record and practice management system with robust interfaces with our local hospital. After hours call (triage only) is shared among sites. Call responsibility is minimal at approximately 1 night per month. Adult inpatient care is provided by a hospital sponsored Hospitalist program, and pediatric inpatient care is shared on a rotating basis by Pediatricians and Family Practice physicians.

We are the premiere primary care provider in the area and provide a very competitive compensation and benefits package and flexible scheduling options.
