1. Scope of Work:

2. The Contractor shall provide services at the Hampton VAMC, one board certified or board eligible, General or Family practice physician who specializes in geriatric medicine.

Specific job functions shall include but are not limited to:

a. Evaluation of all new admissions with completion of admission History and Physical examinations, ordering of pertinent laboratory tests and radiology tests, follow-up examinations, and other duties and responsibilities for patients enrolled in Extended Care Service.
b. Prescribe medications for patients according to the established drug formulary at this facility. Variance from the formulary should occur only when clinically indicated and in compliance with the procedures established
c. Rounding on all Community Living Center (CLC) and Palliative Care Program (PCP) patients shall be done as applicable and a Subjective, Objective, Assessment and Plan (SOAP) note or completion of preprinted templates in Computerized Patient Record System (CPRS) shall be completed on all patients in the time frame defined by the CLC standard operating procedures.
d. Physician shall also be responsible for updating the medical portion of the Medical Data Standards (MDS) requirement for Long Term Care.
e. Physician shall participate in the Interdisciplinary treatment team meetings and rounds in CLC and PCP.
f. Physician is responsible for providing clinical oversight to the Nurse Practitioner (s) working in the CLC
g. The physician shall possess computer skills to access and input typed data on each patient into CPRS utilized by the VA. Four hours of orientation to CPRS will be provided by the VA.
h. Any administrative requirement for referral and/or transfer to another medical facility shall be discussed with the Chief of Extended Care and approved by the Chief of Staff or designee prior to transfer.
i. Services shall be needed forty (40) hours per week from 8:00 a.m. to 4:30 p.m. Monday through Friday, excluding Federal holidays. No payment shall be made for the scheduled half-hour lunch break.
j. Services shall be provided on-site at the Hampton Veteran Administration Medical Center (HVAMC). Services shall include provision of appropriate medical treatment and management of general medical problems of adult and geriatric patients in Extended Care Service. Conditions requiring specialty care shall be referred to specialty clinics within the VA system. There is consultative support available from Internal Medicine, cardiology, pulmonology, hematology/oncology, gastroenterology, nephrology, neurology, Infectious disease, and dermatology, as well as surgical, mental health and rehab services as needed.
k. Contractor Physician shall be provided and held to the standards of quality performance and behavior that are published by the Department of Veterans Affairs (DVA), Veterans Integrated Service Network (VISN) 6, the Hampton VA Medical Center, and the Joint Commission on Accreditation of Healthcare Organizations.
l. Contractor Physician shall complete daily Patient Encounter reports.
m. Contractor Physician shall complete signed notes within 48 hrs.
n. Contractor Physician shall complete evaluation of new patients and schedule follow up visits as required.
o. Contractor Physician shall maintain 100% to 95% accuracy in documentation of patients records
p. Contractor Physician shall treat an average of 10-15 patients per day and 50-75 patients per week.
