Multi-specialty employed physician group in Chicago area is seeking a Family Physician to provide quality patient care and aid in the expansion of home health services into the community. This is an exciting and flexible career opportunity for a Family Physician who is passionate about delivering medical care in the home setting in the community surrounding MacNeal Hospital, located in Berwyn, IL.Chicago Health Medical Group believes a patient relationship is a partnership that spans a lifetime. As the new physician, you will bring healthcare to the patient’s home along with nurse practitioners and physician assistants, and in conjunction with community services, you will provide post- acute services such as home care and hospice services.The goal of the House Call program is to deliver primary care to our patients and their families at home. This includes managing and educating patients about chronic disease and exacerbation in the home setting to avoid unnecessary hospitalizations.
We are seeking someone interested in treating patients in the community setting, there by ensuring a smooth transition of care from hospital to home.Days of operation:Mon-Fri in the field (both part-time and full-time) with an average of 10 visits per day. No weekends. No Call. No rotating shifts. Required are strong assessment skills, laptop documentation, use of technology in the home,excellent communication skills, and the willingness and ability to promote community care delivery at home. Must be BC/BE and have an active (or in process) IL medical license. Spanish language skills are strongly preferred.
