Statement of Work
1. GENERAL
a. Contractor shall provide four (4) qualified physicians for Emergency Department (ED) coverage at the VA Medical Center, Central Alabama Veterans Health Care System (CAVHCS), Montgomery/Tuskegee, Alabama.
b. Physicians referred for consideration shall: be board certified or board eligible for certification in Internal Medicine, Family Practice or Emergency Medicine; have ED experience; have current ACLS and BCLS; be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia; be American citizens (or offer evidence of a green card); and be proficient in the English language. The qualifications of such personnel shall also be subject to review by the VA Medical Center Executive Committee of the Medical Staff, Chief of Staff and approval by the VAMC Director.
c. Physicians referred under this contract shall meet CAVHCS Occupational Health standards for work.
d. The ED physician shall provide initial evaluation, treatment, and disposition for a broad spectrum of illnesses, injuries and psychiatric disorders. CAVHCS is not a trauma center.
e. There is a requirement for physician coverage of shifts from 6:00 a.m. to 6:00 p.m. every day of the week including federal holidays. The work week is Monday through Sunday. Montgomery is the primary site but the physician may be assigned to work at Tuskegee for the 7 day period.
f. A second requirement is for physician coverage of shifts from 6:00 p.m. to 6:00 a.m. every day of the week including federal holidays. The work week is Monday through Sunday. Montgomery is the primary site but the physician may be assigned to work at Tuskegee for the 7 day period.
2. DUTIES – ROUTINE & EMERGENCY
a. The Emergency Department physician shall be responsible for the care of patients who come to the Emergency Department at the Montgomery Campus of CAVHCS.
b. If assigned to the Tuskegee Campus, the contract physician shall be responsible for providing outpatient care for urgent walk-in patients, and for providing necessary care as the house physician for all patient care units at Tuskegee (as needed), to include, but not limited to patient assessment and treatment/management of acute exacerbation and minor medical changes.
c. The contract physician shall be responsible for providing care to CAVHCS personnel who become sick or injured on duty in the absences of the Occupational Health providers.
d. The Physician on duty under the terms of this agreement shall evaluate all individuals requesting care at the facility, including psychiatric care. The Physician on duty shall determine appropriate treatment and/or disposition, and determine whether specialty consultation or admission to the hospital is necessary. When admission is indicated, the Physician on duty shall notify the appropriate admitting (in-house) Attending Physician of the patient and their condition/status.
e. Services shall exceed the level of expertise normally expected of a General Practitioner. In the event of any unusual and/or specialty treatment being necessary, the physician shall contact the appropriate medical staff physician on call for consultation, or refer the patient to another institution as indicated by the facility’s Emergency Department Admissions policies. Specialist consultation shall be made only after clinically indicated work-up and diagnostic testing and/or evaluation.
f. The physician shall be proficient in the following procedures:
1.) Wound closure and suturing, suture removal
2.) Incision and drainage of abscess
3.) Aspiration of cavities – thoracentesis, paracentesis
4.) Insertion of peripheral and central venous lines
5.) Insertion of chest tubes, Cooke catheters, Nasogastric tubes, nasogastric feeding tubes, Foley
6.) Draw venous blood and perform arterial punctures for blood gas
7.) Change gastrostomy tube and suprapubic tubes
8.) Familiar with the working of vascular access apparatus such as those used for chemotherapy and be able to flush them when clogged.
9.) Emergency wound care: e.g. unna boot placement, wound vac. care, wound assessment
10.) Emergency airway management including cricothyroidotomy, endotracheal intubation.
11.) Management of epistaxis with anterior and posterior nasal packing
12.) Hemodynamic stabilization of patients prior to transfer or to admission.
13.) Joint needle aspirations
g. The Physician on duty under the terms of this agreement shall do routine surgical evaluations to include but not be limited to: acute abdomen evaluations, colon obstruction evaluations, insertion of central and A-lines, suturing of lacerations, routine orthopedic evaluations and assessment and treatment of airway obstructions, and insertion of catheters.
h. Contract physicians shall prescribe medications for patients according to appropriate clinical protocols and the established drug formulary. Formulary deviation/variance should occur only when clinically indicated and in compliance with established procedures.
i. Contract physicians shall respond to all medical emergencies including pediatric.
j. Any administrative requirement for referral and/or transfer to another medical facility will be completed by VA personnel and approved by the Chief of Staff or designee prior to transfer.
k. The contract physician shall make physician-to-physician contact calls with private and VA physicians regarding referral of veterans to CAVHCS and make telephone arrangements with appropriate physicians at tertiary care medical centers to arrange for referral of patients to other medical centers.
l. At all times, the incoming physician shall meet with the retiring physician to exchange (handoff) information necessary to assure continuity and coordination. The retiring physician shall not depart the Medical Center until relieved from duty by a CAVHCS physician or another contract physician.
m. The contract physician shall enter all notes, request diagnostic tests, consultative requests, order medications and resulting reports into the computerized patient record system (CPRS) on the day of the visit. In the event of computer system failure, the physician shall maintain sufficient written or typed information to allow for entry in CPRS at a later time. Documentation is expected to meet VHA and medical center policies/Clinical Practice Guideline and Joint Commission/HCFA standards for documentation and subsequent coding. The physician shall have a strong working knowledge of computers/skills to access the computerized patient record system (CPRS) utilized by the VA. CPRS orientation will be provided to the new provider, but this training cannot compensate for limited exposure to computers and poor data entry skills.
n. The contract shall participate in fire and disaster drills.
o. The contract physician shall complete appropriate evaluation and paperwork for special incidents such as medication errors, restraints, falls, injuries, adverse drug reactions, etc.
p. Contract physicians shall participate in administrative and quality improvement activities to include a focus on customer satisfaction, length of stay and all VA performance measures
