​Opportunity:
As the incoming Transplant Physician Assistant, you'll join coordinate and organize the evaluation and care of patients for possible kidney transplant. Position would coordinate follow-up outpatient visits, as well as inpatient and outpatient care. You will also actively participate in the procurement process for organs transplantable to waitlisted patients as well as manage currently active patients on the kidney transplant waitlist and communicate with transplant team any necessary waitlist status changes.

ESSENTIAL JOB FUNCTIONS:
1. Supports the Code of Conduct. Demonstrates behaviors consistent with organization mission and goals.

6. Plan, coordinate and implement administration of in-patient and out-patient services: a. Plan, organize and provide for potential living donors. b. Coordinates with other members of the Transplant Team during Selection Committee to facilitate representation of potential living donors. c. Facilitates development of living donor education. d. Integrates state of the art practices into the plan of care
7. Knowledge of donor selection criteria for transplant services.
8. Knowledge of policies, procedures and standards required for living donors.
9. Serve as a resource for hospital staff regarding the care of transplant populations: a. Formal presentations, unit based in-services, resource materials b. Publish and/or present at national meetings: abstracts, posters, research data c. Communicates departmental scope of services to employees
10. Coordinates information communication among: a. Case managers b. Medical staff, nursing staff, ancillary staff
11. Ensures the coordination of the clinical aspects of the potential donor/recipient.
12. Specific responsibilities during candidate phase: Participates in education; Assists in the evaluation and selection of potential recipients; Monitors pre-transplant timelines and communicates any issues or concerns to appropriate transplant physicians, transplant coordinators and transplant social worker.
13. Specific responsibilities during transplant/inpatient phase: Maintains communication with recipient and family. Assists in the procurement/transplant of recipient/donor. Contributes to their education regarding transplantation; Monitors donor/recipient during inpatient phase and prior to discharge and communicates any issues or concerns to transplant physician, transplant coordinators and transplant social worker.
14. Specific responsibilities during recipient/outpatient phase: Follows-up with living donor as well as recipients post discharge. Communicates all issues and concerns to appropriate transplant physician, transplant coordinators and transplant social worker.
15. Monitors all patients’ emotional, spiritual and psychosocial status.
16. Provides data coordinator with all information necessary to meet UNOS and Medicare regulations as relates to living donors.
17. Fosters cohesiveness and unity among the multidisciplinary care team.
18. Maintains open communication between the multidisciplinary care team to identify challenges and resolve issues and address process improvement opportunities in a team oriented approach.
19. Participates in multidisciplinary care team planning and collaboration including selection committee.
20. Applies knowledge of the principles of growth and development over the life span to identify requirements relative to age-specific needs.
21. Assess potential learning problems (language barrier, readiness to learn, etc.) and structures teaching sessions to meet individual needs.
22. Participates in transplant and organ donation awareness initiatives.
23. Keeps abreast of research/literature/developments in transplant specific to rights and advocacy for living donors and attends appropriate continuing education institutes and conferences.
24. Maintains a working knowledge of regulatory agency policies and procedures.
25. Provides Pre, Intra and Post follow up visits.
26. Other duties as assigned.

The successful candidate will be a certified PA/NP with 3+ years of experience in Transplant, first assist experience preferred.
