Job Description
Diagnosing and treating acute and chronic health care problems. Performing comprehensive history and physical exam. Providing daily assessment in an inpatient setting. Prescribe medications and surveillance of medical regimens for stabilized chronically mentally ill persons, recognizing when to refer the patient to physician or other health care team member. Working collaboratively with the Interdisciplinary treatment team (Patient, Psychiatrist, nursing, Social Work, Psychology and other ancillary service providers). Participating in educational activities. Providing patient and family health education with a focus on self-management, prevention, and wellness based on the patient's goals. Formulate and implement plan of care which may involve various treatment modalities. Use an Electronic Health Record system to obtain patient information, document care, order lab tests and medications. Consult with internal and external customers including hospital staff and outside providers as well as area wide mental health professionals and organizations.
