The Grand Junction Community Living Center is a 30-bed extended care facility located on the campus of the Veterans Health Care System. The services offered include skilled nursing care, inpatient rehabilitation, long term care, respite stay and hospice care at the end of life. The PA/NP is expected to provide comprehensive primary inpatient care for veterans admitted to the CLC with supervision by the CLC/Hospice & Palliative Care Medical Director. This position would entail working closely with members of the CLC Interdisciplinary Team and also be an active member of the Palliative Care Consultation Team. Responsibilities include but not limited to: Performs admission H&P, discharge summary, admission orders, initiates and follows diagnostic work up.
