Position Highlights:
Responsibilities:
- Provide primary care to patients while receiving rehabilitative care in our specialty hospitals
- Collaborate with treatment team members for medication management
- Implement an individualized plan of care for patients' goals
- Complete Medicare required visits during hospital stay
- Complete acute, routine follow up, and discharge visits, and transitional care
- Make recommendations on DME, home health services, or outpatient services post discharge
