Site Physician (Internal Medicine/Family Medicine) - Queens, NY

Shifts: Mon - Fri (8:00 AM - 5:00 PM)

Job Description:

Under the direction of the Chief Medical Officer, the Site Physician is responsible for the professional, organizational, and administrative aspects of providing quality medical care to the patients.

Roles and Responsibilities:

• Direct the medical care and treatment of patients and have primary medical responsibility in overseeing and supporting the care rendered by other members of the interdisciplinary care team.

• Collaborates with site NP community-based based primary care providers on the shared population.

• Identifies opportunities to improve the quality, cost, and utilization of care for the site participants.

• Carries a caseload of patients as PCP and participates in direct medical care in the Diagnostic and Treatment Center.

• Leads the medical presence during the start of care events.

Qualification/Experience:

• Graduate of an accredited school of medicine

• Board Certified or Board Eligible in Internal Medicine or Family Practice

• Minimum Experience in Geriatric Medicine

Licenses/Certifications:

• Current active unrestricted license to practice medicine in State NY

• Active DEA License
