Job Responsibilities

Provide comprehensive primary care services to patients of all ages, including preventive care, routine screenings, chronic disease management, and acute illness management.
Establish and maintain strong patient-provider relationships based on trust, empathy, and mutual respect.
Conduct thorough medical evaluations, diagnose medical conditions, and develop individualized treatment plans in accordance with evidence-based guidelines.
Coordinate care with specialists, allied health professionals, and other members of the healthcare team to ensure integrated and holistic patient care.
Utilize electronic health records (EHR) system for documentation, prescription management, and communication with patients and colleagues.
Participate in quality improvement initiatives, peer review activities, and continuing medical education to maintain and enhance clinical knowledge and skills.
Provide patient education on preventive healthcare measures, disease management, medication adherence, and lifestyle modifications.
Collaborate with community organizations and resources to address social determinants of health and improve health outcomes for patients.

Qualifications

Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.
Board certification or eligibility in Family Medicine or Internal Medicine.
Active, unrestricted medical license to practice in the state of New York.
Minimum of three years of clinical experience practicing family medicine or internal medicine in Queens, NY or a similar urban setting.
Strong clinical skills and knowledge of evidence-based primary care practices.
Excellent interpersonal, communication, and leadership skills.
Ability to work effectively in a collaborative, multidisciplinary team environment.
Commitment to providing culturally competent care to a diverse patient population.
Proficiency in electronic health record (EHR) documentation and computer skills.
