A health services network in Texas is currently seeking a Board-Certified Nurse Practitioner to join their clinic in El Paso.

About the Opportunity:

  • Schedule: Monday to Friday
  • Hours: 8am to 5pm
  • Setting: Clinic
  • Patient Population: 65+
  • Patient Volume: 12 per day

Responsibilities:

The Nurse Practitioner will be responsible for:

  • Provide patient facing ACUTE and other medical evaluations to individual members
  • Encounters for assessment
  • Post-hospital discharge stabilization, and follow up and consultation with stabilization coordinators for state-wide panel of members.
  • Evaluate and document accurate patient information to provide expert health assessment and treatment of complex conditions via telehealth platform including interpretation of vitals, medication reconciliation!
  • Coordination with primary care providers, specialists, and hospital-based providers.

Qualifications:

  • Licensed to practice medicine in the State of Texas
  • Required Certifications Board Certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC,)
